PCHC is pleased to announce a new, fully-equipped dental clinic opening on
April 29, 2011 at PCHC.
Eligibility: To be eligible, children and youth must be 17 years and under, their families must be residents of Ontario, must not have dental insurance or be covered by other provincially funded dental programs e.g. Ontario Works, and the family’s adjusted Family Net Income must be $20,000 or less. Proof of income, date of birth, and address is required. However, if families do not have documents to meet these requirements, they can self-declare that they are eligible and get someone in their community (e.g. a doctor, nurse, a well known community worker, a leader of a faith institution etc.) to be a guarantor that they are eligible for the HSO program. The guarantor will be required to sign a form on behalf of the family, confirming eligibility. You may enrol in the HSO program in person at the dental clinic at PCHC or at any Toronto Public Health dental clinic. Once enrolled, you will be issued an eligibility card that you can take to any dental care provider (private dentist or hygienist) or make an appointment at the PCHC dental clinic, to obtain dental care. The new dental program, which is also known as the Healthy Smiles Ontario (HSO) program, was introduced in October 2010 and funds dental care for eligible children and youth 17 and under. The objective of the HSO dental program is to allow children and youth to get regular dental care to prevent cavities and gum disease. A healthy mouth is important to the general health and wellbeing of children and youth. A healthy mouth also contributes to children feeling good about themselves, being able to eat healthy foods and feeling comfortable in social situations. We also know that healthy children and youth do better in school. The HSO program covers regular visits to a licensed dental care provider, i.e. a dentist or dental hygienist. Services covered include examinations, x-rays, fillings, extractions,cleaning, and root canal treatment. Orthodontic (e.g. braces) and cosmetic services (e.g. teeth whitening)
are not covered. The official opening of the PCHC dental clinic will take place on April 29, 2011 on Community Health Day .Please visit us to see the new dental clinic and the programs
